Annual Re-Enrollment (Sponsor Level)

Generates Re-Enrollment form for all active participants for all active sites.

  • Begin by logging in as a sponsor admin.
  • From the main dashboard, select the tab for Reports

  • From the dropdown, select Income and Enrollment Form Reports

  • Click the hyperlink for the report labeled Annual Re-Enrollment
  • Choose the Effective Date of Enrollment Renewal
  • Choose the Expiration Date of Enrollment Renewal
  • Click the green "Run Report" button to generate a hyperlink
  • Click the Download Report hyperlink to download the report as a PDF
