Income Certification Address Logging [Report - Sponsor Version]

This report contains the information for the required CACFP regulations for documentation of enrollment. This includes information on each child's normal days and hours of care and the meals normally received while in care. Regulations also require that income forms be distributed to families with children enrolled in child care centers and adult day care participants in order to determine reimbursement rates.


For sponsors that choose to collect this information using Ready Records, this report will provide a list of the form requests that have been sent, their status and the IP address of the device used to complete the forms.

  • Begin by logging in as a sponsor admin.
  • From the main dashboard, click the button for Reports.
  • From the dropdown, select Income and Enrollment Form Reports

  • Scroll to the Ready Records sub-category.
  • Click on the hyperlink for the report labeled Income Certification Address Logging.
  • Choose the date range for the report.
  • Click the green "Run Report" button to generate a hyperlink
  • Click the Download Report hyperlink to download the report as a CSV file.
