Income Certification Address Logging

The CACFP regulations require that documentation of enrollment be collected, including information on each child's normal days and hours of care and the meals normally received while in care. Regulations also require that income forms be distributed to families with children enrolled in child care centers and adult day care participants in order to determine reimbursement rates.
For sponsors that choose to collect this information using Ready Records, this report will provide a list of the form requests that have been sent, their status and the IP address of the device used to complete the forms.

- Begin by logging in as a sponsor admin.
- From the main dashboard, select the tab for Reports

- Choose he date range for the report
- Click the green "Run Report" button to generate a hyperlink
- Click the Download Report hyperlink to download the report as a PDF
