Missing Forms Detail Report
This report helps sponsors to identify missing or expired income and/or enrollment forms.
- Begin by logging in as a sponsor admin.
- From the main dashboard, select the tab for Reports
- From the dropdown, select Income and Enrollment Form Reports
- Choose from the available report options:
- Month/Year
- Missing, Expired, Expiring OR All
- Enrollment OR Income Forms
- By Site OR Monitor
- Click the green "Run Report" button to generate a hyperlink
- Click the Download Report hyperlink to download the report as a PDF
