Food Allergy Label Report

Facilities that use a central kitchen or a vended meal provider may need to have food service staff make accommodations for special dietary needs. These labels can save time as central kitchen or vended meal providers can print and apply the labels on the foil or plastic wrap that covers the modified meal to designate the location and participant for which is is intended.


This report will generate a printable label for any participant in the system with a special diet form with the following information: 

  • Name
  • Roster (Classroom)
  • Any information listed on the participant's special diet, including 
    • Special Diet summary
    • Food items to omit
    • Food items to substitute


There is also space on the label for the food service staff to hand write additional information.


These are formatted for labels that are 1-1/3” x 4”.

  • From the main sponsor dashboard click on the green tab for Reports. This will take you to a screen to select the report category.
  • Select report category Food Service Operations Reports.
  • Click the hyperlink for the report titled "Food Allergy Label Report".
  • Enter the date range for the report.
  • Select specific sites or all sites.
  • Select specific meals or all meals.
  • Click the green Run Report button to generate the report.
  • Once the report has been generated, click the blue Download Report hyperlink.
