Add a Custom Site Visit Question [Feature]

This feature allows users to create their own custom questions on the visit page. It is helpful for sponsors that do not need to fill out an exact copy of their state agency monitoring form and can instead answer the same questions within My Food Program.

Use the guide below to decide which option is right for you:

To use this feature:
  • Login as a Sponsor Admin User.
  • Click the Setup button.
  • Click on Site Visit Customization.
  • On this page you will see a list of existing site visit questions. To add a new one, click the blue Add button in the upper-right corner.
  • Enter the text of the question you want to ask.
  • Indicate if we should display this question by checking the Active box.
  • Indicate if you would like to Mark as Important so that the question will be displayed with a red background to emphasize it.
  • Select the type of question. Choose from the available four question types:
    • Short Text
            • Long Text
            • Single Choice - Dropdown
              • Click the blue Add Answer button to add the acceptable responses.
            • Single Choice - Radio Buttons
              • Click the blue Add Answer button to add the acceptable responses.
            • Multiple Choice
              • Click the blue Add Answer button to add the acceptable responses.
            • Date
              • Option for multiple dates displayed in columns
            • Signature
              • All signature questions automatically include a box to draw the signature, a space for a typed name and a date/time stamp.
          • All active custom visit questions will now appear on the visit screen in My Food Program.
