Parent Consent for Home Delivery Report
This report consists of the list of participants and their most recent documentation of parent consent for delivered meals. This spreadsheet includes site name, parent name, child(ren) names and effective and expiration dates of the forms.
- Login as a sponsor admin user.
- Click on the green tab labeled Reports.
- From the dropdown, select the category Monitoring and Training.
- Click on the report titled Parent Consent for Home Delivery Report.
- Select Specific Sites or Select All Sites.
- Click the green Run Report button to create a hyperlink.
- Click the Download Report hyperlink to download the report in XLS format.
