Apply Electronic Signature to Meal Count Forms After Claim is Submitted

My Food Program has the option to collect and store electronic signatures for providers or centers. These signatures can then be populated onto several reports:


1. Daily Meal Count with Attendance Report

2. Weekly Meal Count with Attendance Report

3. Monthly Meal Count with Attendance Report

4. Time Distribution Report

5. Staff Hours Detail Report


The signature that is applied is the site manager/director that is marked as the primary contact.

The date that is applied is the date that the provider or center submitted their claim to their sponsor.


Check this box if you want to require your provider or centers to create an electronic signature that will be applied to the reports listed above.


Leave this box unchecked if your state agency does not require signatures on forms OR they require pen-and-paper signatures.
