Enrollment Form Expiration Date Policy

This policy refers to the date that you would like us to pre-populate on the enrollment form screen. Sponsor users are always able to change the enrollment form expiration date after pre-populating the dates.

Select from these options:

• One year from effective date
• One day prior to one year from effective date
• End of the month one year from effective date
• End of the prior month one year from effective date
• Specific date
