Only Enrollment Form Included in Participant Registration Download

My Food Program can generate the following forms pre-printed with household and participant information:

1. Income Form

2. Enrollment Form

Check this box if you are a sponsor of family child care homes and therefore only want an enrollment form since parents enrolled with Tier I providers are not required to complete an income form.

Leave this box unchecked if you are not a sponsor of family child care homes and you want participant registration packets to include income forms as well as enrollment forms.
