Participants Enrolled in Multple Sites
This assists sponsors in identifying active participants with an exact match in multiple site on:
- First Name
- Last Name
- Date of Birth
This report is supplement to our option to automatically disallows meals for duplicate participants and the report Participants with Duplicate Meals in Multiple Sites.
- Login at the Sponsor level
- Click on the green tab for Reports
- Select the "Monitoring and Training" category from the dropdown
- Click on the "Serious Deficiency" report from the list.
- Select to run the report by name or by site
- If you select by name, select whether the match should be just participant name or also on primary guardian name
- Click the green Run Report button to create a hyperlink.
- Click the Download Report hyperlink to download the report in PDF format.
